Executive Committee Feedback

As the annual evaluation process for faculty members approaches, I have developed the following process with advice from the Faculty Council for you to provide feedback on the performance of members of the Executive Committee taking into consideration the earlier work of the Organizational Procedures Committee. The details on that process have been sent in a email to the all-lib listserv. The forms for the process can be found on the Office of the Dean SharePoint site under Executive Committee Feedback. If you wish to provide feedback on your division’s leader or any of the other members of the Executive Committee, please use either the form for division members (if you work in that leader’s division) or the form for non-division members (if you do not work in that leader’s division). Please submit individual forms for each leader. Those may be sent to me electronically or in print form. They must be signed for consideration but will be held in the strictest confidence. They will not be shared with the members of the Executive Committee. The deadline for submission of the forms is December 15, 2008.
